Initially I may blame myself, someone else, or fate. But soon I accept that blaming solves nothing.
Then I try to understand the cause of the situation, and what I can do to improve it. If I can improve it, I do. If I can't, I accept that I just have to accept it and move on with my life.
I don't think that everything bad that happens is someone's fault, or that it is destined to occur. Some things are just random occurrences, or happen to us because we are living organisms, like some diseases or medical crises. While I recognize that the latter may be influenced by life style, eventually we will all die of something, and we cannot predict what or when.
But blaming just gets in the way of a solution. I would rather focus on the solution, not the fault-finding. That's a lot more constructive.

Well, it would vary depending on the problem, but I wouldn't get as much caught up into who's fault it is as to getting it worked out because that would be most important. If someone caused me regular problems I would stay away from them.
