Not really, maybe they are concerned. Not everyone has the gift of tact or the best way to express their thoughts. So what they say comes out wrong. I usually make a playful sarcastic comment such as "Well thank you for that insight it was just what I needed to hear today to make me feel better"

No not at all as sometimes I do get tired as everyone does and it shows. I don't think that anyone is trying to be insulting by telling you this and you really shouldn't get jumpy or sensitive about it. Think about it, if you felt like punching someone, perhaps you really are tired. No something has to be a genuine insult such as you are stupid or you are ugly before I consider it as an insult. Pretty soon nobody will want to be around you if you get mad at little things and believe me, that is little.

No, not necessarily.
Sometimes others are able to see when we aren't feeling good long before we even feel it.
I think it's nature's way of letting us know when we don't look or feel our best.
When somoene tells me I look tired, then I'll say, "Really?!" and go look in the mirror.
Maybe the see something I don't. Who knows and who really cares.
I'm not a vain person so I really don't take it so personally.

No, when someone says that, I usually am. I get these huge bags and dark circles under my eyes that are a dead give-away to my state of tiredness. Who wouldn't notice them?

I just explain that I tied one on the night before, which is usually true. That makes the person who asked feel pretty uncomfortable.
ETA: Or I give them a wink and say it was a looong night, if you know what I mean....

If I really do feel tired than it doesn't bother me. My boss sometimes says "Do you feel alright, you look sick." When I feel completley fine, that is when I would like to give him an uppercut to the jaw!!
