No form of contraception is 100 percent effective, so yes there is a possibility she can become pregnant. Its less likely but it could still happen.

OF COURSE!
NOTHING is 100% effective except for abstinence.
So even using condoms, you can get pregnant! They are only 99% effective when used correctly

the possibility is slow, unless the condom has been broken and your gf is fertile.. if both are negative then the it is highly improbable that your gf will not get preggy. good luck!
