Cloth pads, usually. In some areas they used natural fibers like moss, grasses, or cotton. And in some times and eras, they (brace yourself) just bled into their clothes.

You've heard the expression "on her rags"? That should give you a hint. Basically they would use old rags that didn't matter if they got stained. Whether or not they were reused I don't know.
