I'm looking to dampen sound from the main level of my home, and the basement.

hardwood. I always go with the hardwood. Does a great job of dampening.

Depends. Livingroom / familyroom and halls carpet. Bathroom tile ~ kitchen hardwood.

i prefer hardwood but i think carpet will dampen sound better.

Non-faced insulation first and then do what ever flooring you want!
But if this is the basement that you want to put flooring in don't use hardwood cause if it ever floods its trashed.

For sound damping, carpet is best; a good underlayment on a wood floor could work. Tile is too echo-y.
