It doesnt always do it so its not poor construction for the most part.

I have a grate at the top no animal can fit in the chimney.

unless its an insect.

The best thing you can do when you light a fire is burn a bit of paper to warm the flue and create a decent updraft. When it is substantially cool out, it can be difficult to get an updraft started and the result is a backfeed of smoke into the room. I would bet that you are getting the most smoke when the flue is the coldest and the updraft is fighting the hardest. A nice quick burst of heat from paper burning will warm the flue enough to get the updraft started.

A) don't use the fireplace.
B) There may have been a piece of creosote that broke loose and tilted sideways and, blocked the flue.
C) Look up the chimney with a mirror and, or a flashlight, you should see open sky.
D) For the most part unless it's dirty, or something is amiss, I would find out what the problem is before I used it again.

Since you said it doesn't always happen I'm ruling out the damper.
It is very possible an animal has made a home in your chimney! They will get out when the smoke gets bad, but by that time, you may have some smoke in the house.
Make sure you have a chimney cap (wire grid) to prevent birds, raccoons, squirrels and other critters from entering.

If your damper is open, you've got some drafts from other places in the room/home causing negative pressure pulling the smoke into the room. They do smoke tests to find these types of problems

Your chimney may need to be cleaned, plus if you are getting high winds you could be getting down drafts in the chimney that are causing the smoke to come into the house.

try taking a roll of news paper lite the end of it.Reach high in the chimney and warm it up witch will cause a good draft.
Your house may also be in a negative may need make up air

is the fluke open?
does your chimney need cleaning?
just use candles is the fire place.
