I have graduated from 3 universities (for professional degrees) and I was never required or even asked to take a physical. Unless you're attending a military school (West Point, VMI, US Naval Academy) I would be curious to to know why a college requires a physical exam.

Remember, you're there to get a complete picture of your health, and there isn't much that you've got that your doctor hasn't already seen. There's a good chance that you won't be asked to completely undressed. If you are, and you don't feel comfortable, just ask to stay dressed to the point where you are comfortable.
For a college physical, I can't remember ever having to take off more than my shirt so the doctor could listen to my heart and lungs. Yes, I got the "turn your head and cough" test, but it was brief and I didn't have to completely undress.
As far as what happens, there's nothing to worry about. Most physicals check your height and weight; have your blood pressure, pulse, and temperature taken; might have your eyesight and hearing tested; check your eyes, ears, nose, throat; listen to your heart and lungs; feel around your stomach and abdomen. You might have the "turn your head and cough" test, but remember, the doctor has seen thousands of patients and it's just another test. And that's pretty much it. Nothing to worry about.
Good luck, you'll do fine.

Unless you're taking part in athletics, a college wouldn't require a physical. Even if you are taking part in athletics and a physical is requested, you certainly will be able to submit the results of your last physical from your primary physician. Along with the results of your last physical(given within the last year), there will be some routine questions, but nothing exhaustive. If colleges want your athletic prowess, they will let a lot of stuff slide, believe me.

you will more then likley have to strip down to your underwear and probably have to reveal your genetals for a quick moment but not long and they are done in private. and the person giving you the physical is a professional and has done their job many time so theres nothing to worry about.
