It's ok warm if it's just opened or out of the packets at McD's. I'd rather have it cold, though!
You must refridgerate it to keep it from spoiling. If you've ever smelled rotten ketchup, you'd understand. I used to have to fill/refill the bottles when waitressing as a teenager. In the summer it would spoil FAST. SO GROSS. I can't eat ketchup to THIS day without giving it the smell check!!

you dont have to put it in the fridge. and all putting it in the fridge does is slows down spoilage.. granted i wouldnt put it out in the sun for too long either
Heinz is not required... it was only recently when they made the "fridge door" size did it even say anything about putting it in the fridge.
If you are worried about it not being cold.. dont ever eat any ketchup at ANY restaurant. I've never once seen or heard of one that refrigerated their ketchup.
I've gone 25 years of eating warm ketchup.. my in laws used to tell me how sick it would make me until I showed them their bottle of ketchup and how it doesnt say must be refrigerated.

I prefer warm, but I like to refrigerate my ketchup just to be on the safe side. I'd rather not risk food contamination and have it cold, that to leave it out and risk it.

Room temperature is the best. I enjoy when I can get Heinz out of the packets at a take out and put the ketchup directly on my burger.

I much prefer my ketchup cold. I think it actually says refrigerate after opening on the bottle.

Do you mean you warm it up?? That's odd. I like it room temperature. Slightly chilled is fine also.
