You don't HAVE to buy things..MAKE them something..I know that sounds a bit like kid stuff, but you can make cookies and hand them out maybe.(my mom used to do this).. or any kind of dessert that you're really good at making..
If you can, you can at least get people Xmas cards,even if they are just the $.99 ones at Walgreen's/CVS.
or be creative and make cards.. why not? It's Xmas,people will appreciate it .
A huge thanks and much appreciation may be enough for some..
It'll be OK.. .
Focus on caring for yourself first of all though.. YOU right now are most important for the sake of the baby, since you are high risk. Don't stress about this, it'll all be OK.

By a large pack of inexpensive gift cards and stick a candy cane onto them. Write a personal note in each. You would be amazed but people just want to know that you acknowledge them.

It should definately be understandable to them that obviously you can't afford to buy Christmas presesnts this year. Maybe just make a little Christmas card/ thank you card just to show your appreciation for the gift you received from them! Merry Christmas!!

Get over it. People are buying you things because they love you. If theyre giving you things and want something in return thats their own fault for being so freaking shallow, you dont need people like that in your life in the first place.
Just enjoy being pregnant and bask in the love of your friends and family.

Once you have finished your course of pregnancy you should start to work and 'pay back' the people that got you gifts .. However for know .. Just thank them and they should understand that you can't finacially support yourself.. By also buying gifts for others..
