For the first one, I suspect you mean
4/(x+3) + 5/(4x+12)

If true, multiply the first term by 4/4 (that's 1) and get
16/(4x+12) + 5(4x+12). Now you have a common denominator.

21/(4x+12)

B) -5/3y-15
