yes they are not 100% effective even if you do everything right. If you don't leave space at the tip it could come out of the top.

No unless it broke or slipped off inside you or a hole was poked in it there is almost no chance at all that could happen, I think your safe. But if you are absolutely not wanting pregnancy then you should take the pill and use condoms. Just in a month of being off the pill and using condoms after having my son my husband and I had 3 break on us! Crazy yet scarry!

If it comes out the top, it did break! Nothing is 100% effective, but it's very unlikely that you will get preggers using a condom that did not break.
