I read something about a heavy heart in a poem recently, and also heard it in a song. I cant find anything on it on the internet. What does the phrase 'heavy heart' or 'having a heavy heart' mean?

Yes, it means to be very sad. The word has more than a poetic meaning because such a state of mind is often accompanied by a tightness in the chest as if the heart were swelling (growing heavy.)
