It means a mistake...faus paux...oops...that someone just humiliated themself...however you want to phrase it.
It basically means that someone screwed up.
Personally, I think the term "snafu" is a true oxymoron...how can something be normal if it is all f*cked up? Unless, of course, that's the way said situation always is.
