Well, if they are a friend, I would just straight up tell them. And if not, I would offer them a mint.....then when they go to take it, just tell them to keep the whole box.

No, don't just ignore it for 4 weeks, you're not helping him, because he'll go on somewhere else with the same problem. Just talk to him on your own and explain gently but very clearly that if he goes on with the same table manners he is going to offend many more people. Explain that this is how you hold your cutlery, this is how you eat, you're not at a trough, you're at a table, manners are important, ask him if he has ever observed how most other people eat, etc ... I know people with terrible table manners but then I've eaten with their parents and I can see exactly where the example came from ...

Depends on who it is. A close friend is easy; step back a bit and say, "Please do me a favor and go brush your teeth." A person who is a co-worker or mere acquaintance is a lot harder.
My mother used to have the knack of saying such things. She'd start with, "I know people don't usually like to mention such things, so you probably don't realize that . . ." It worked for all sorts of things, like bad breath, slip showing, body odor, etc. But she was such a gentle, kind lady that I doubt anyone ever criticized her for it.

It would depend on how close of a friend they are, if they are close I would simply tell them! You need to brush your teeth and use mouth wash! If it was someone else I might suggest some breat mints I hate gum chewers!

It could be medical and they probably already know. Just offer gum or something. I always try to chew gum because I have HUGE tonsils that really need removing.

I'm not sure there is a best way to do this. I would have some mints or spray on hand so they can fix it right away.
Do they have stomach or teeth problems?

You can tell them that you just went to the dentist and ask them if they have been recently.... tell 'em that your dentist has you worried about gingivitis...and you only get one set of teeth... :) Mints and gum are only a temporary fix.

If I don't know them well, I'd offer a stick of gum, tic-tac or something. If I do know them well I offer the same thing, but add, "no offense but you need this."
