To be honest it depends on what type of world you like and what type of gameplay. I prefer action-rpgs like Diablo II or Nox where it's more like a top down shooter with rpg elements.
Games like Everquest II and Vanguard are known for being more technical than World of Warcraft, but more people play WoW than either of them across the planet.
There are games which don't have monthly subscriptions like Guild Wars and Neverwinter nights. There are also games like Everquest II, World of Warcraft, and Vanguard which require monthly fees.
If you don't like fantasy there are space based games like Eve Online.

Ok... i just recently took a break from finalfantasy XI (11) because i overplay it. I tried Everquest and i didnt like it much... kinda boring. You might like it tho. Also, my brother told me it played ver similar to WOW and... im against wow... i think its overrated. Ive been playing final fantasy for like 5-6 years now tho. The number of players online have decreased a little bit but thats because of the introduction of alot of other MMORPGs... its an amazing game. I will tell you in advance that it will be boring but if you tough through the first couple days and even research a few tips (ffxiclopedia.com) you'll get hooked. Alot of great ppl play that game too, you'll have a great time.

well, I have a couple suggestions, if you want to pay each month.
Ultima Online (this was my favorite game- Nothing compared to this one. its a medevile game, with animal tamers, warriors, mages, etc)
Dungeons & Dragons Online (my parents were into this)
Everquest II (similar to the above, largely played, good graphics)
Dont know any free games related to Runescape, though.

well if i had to suggest something then i would say World of Warcraft but if you don't like paying for games then i would say Perfect World (MY) or RF online and if you don't like those you can also look for an mmorpg list such as http://www.freemmorpglist.com/ that's the site i use

there are alot of games out there
it just depends on how much your willing to pay
if you want free then all i know of is runescape
but world of warcraft is VERY popular
