directly & honestly, basically just like you wrote it here.
If you want to stay with him, make suggestion for improvement. Say something like, "honey I love you but, you're a premer and are shite at foreplay and have a small dick, but as I love you for more that just sex, here are some ways to increase my pleasure..."
If you don't, list them as reasons you're breaking up with him (which will completely crush his ego).

If you keep telling him that he is amazing in bed then you are the problem....don't tell him he sucks, tell him how he can improve. If you like him then get him to learn how to do it the right way.
