Why does the doctor's office ask for my SS# and Driver's License number? Do I have to give it to them?

For positive identification. No you don't; and they don't have to accept you as a patient; you can look for another doctor.

For identification purposes,
a) in case you don't pay your bill, and
b) in case you're going to multiple doctors under different names to get drugs.

If you don't give them to him he probably won't accept you as a patient, and even if he does he won't be allowed to prescribe you any drugs.

Depends on if you are admitted or not, for a check up, No for the emergancy room, yes most of the time

It is used to find you if you fail to pay your bill.

As much as I dislike illegal aliens in the US, it would also prove identity and citizenship if we decide to prevent illegals from getting health care.

yes, on the SSN. they need to be sure you are who you say you are. too many uninsured people out there. they are just being sure some one did not steal/find you wallet and are not trying to get health care in your name.
