It's a PDF that is not fillable, and we should be filled by pen, and then printed.

But there is a small box for name and signature that says "Please print", I wondered do they mean to write in 'All capital' or 'noraml' by this phrase.

Print simply means to write it normally using capital letters and lower case letters, but not in script form. Print is the type of writing you do before you learn how to write in script in school.

In the small box where you have to write your name, and it says please print, they mean to print your name. Where it says signature, that's the only place they want you to write in script. Write your name in script there.

I don't think it matters whether you use all capitals or not when you print your name. All they want is something legible (readable) so they can tell how your name is supposed to be spelled.

When many people sign their names in cursive, it's impossible to read their signatures because of poor handwriting or what's known as "the executive" signature (like a doctor's). As long as your printing is fairly neat and readable, I wouldn't sweat it.

Print means the type of writing you are looking at right now. Cursive is the kind of writing you use with your normal handwriting. They want you to "print" not "cursive." When they ask for signature, they want it in cursive, your normal signature. Sometimes they want it in both forms because signatures are often hard to read.

Write in block capital letters - ie do not join any letters.
