nope. And you will notice in most factory settings the washroom is located near the lunchroom(aka where the kitchen sink is so = plumbing). So you "do it" during your breaks....not during work time. Your body works that way....if you are busy or running away the body knows "now is not a good time" to have a dump.

Laws on this vary by locality. In many places, the only requirement is to provide a restroom "on premises". In others, this will be governed by some sort of building or occupancy code.

No in general, unless you are disabled then your employer must make reasonable accommodation.
Check your local ordinance though. It could be included if the city or township things that is good for business.
