Think of the picture it paints.
If your feet are on the ground, you are standing upright, able to walk around, doing what active, capable people do.
If you have your head on your shoulders, where else should it be? Of course, it is where it belongs, and helps your body and mind work well. It keeps you alive and functioning.
It implies that your able to think straight, to do well, to see what is real.
It is meant as a complement, although it might also mean you are boring and shallow, because you don't see the "dream" side of life.

The person is living in the real world. If the person has their heads in the clouds they are dreamers. They do not live in the real world.

For example if someone thinks they are a going to be a TV chef and they can't even cook they would have their "heads in the clouds." Now, if they were going to culinary school to be a chef at a local restaurant and later open their own restaurant they would have their feet on the ground and head on their shoulders.

It is a complement that means someone is grounded in reality and has good common sense. To a lesser extent, it can also be taken to mean that the person being spoken of is heading somewhere in life.
