Go to an arts & crafts store and buy some materials to make her a scrap book. In the scrap book you can include pictures, quotes, and written memories that the two of you share. She will love it because it will hold sentimental value and it comes from the heart; it will have much more meaning then anything store brought because you took your time to make it meaningful.

Call her Christmas eve and tell her to get dressed you are going to take her out....arrive in a Limo wearing a tux or a gorilla suit (your choice depending on how brave you are... you can rent them) and carrying a dozen roses and something gold wrapped in a box. Take her for a drive around town to see some Christmas lights, have champagne and some fruit or other snacks to nibble on, or ask the driver to go through McDonald's for burgers....it is the oddity of taking a Limo through a fast food place that is so charming...enjoy an hour or two together and renew all those romantic feelings you had years ago.

Get her something from the past . Something from the place where you two first met . From where you had your first date with her . From where you first told her you love her . Take her out to a nice romantic dinner , dancing . And a nice walk on the beach , if there's a beach where you live . When you get home have candle's lit around the bedroom with some music and some champagne ( if ya like champagne) . Tell her everything that she means to you from your heart . What about a mystery flight, a nice weekend away, a night out on the town then stay in a nice hotel room but dont tell her just organise the hotel room pack some of her clothes you know she likes to wear and suprise her. Tell her you have to go into the city and then drive in and suprise her

I agree with the spa, but other ideas
gift cards to favorite stores (not romantic, but my favorite gift from anyone)
dinner and a show
an evening at home with you alone. Think: a home cooked dinner, you draw her a bubble bath and wash her back, movie and popcorn... then your on your own
ask her girlfriends... We girls talk, just not to you... Don't want to look desperate!

a day of pampering is a wonderful gift, go out and get her a gift certificate to the beauty shop to get her hair done and nails and a facial and get her another gift card to a nice store to pick out a nice dress or outfit and then top it off with another gift card for dinner , and when you get to dinner and shes looking lovely , tell her you were touched by an angel all them years ago and this year you wanted her to be treated like one for a day.

a scrap book or a collage of your years together. this will just show her how much importance you've given to the life you have shared. and of course, the effort is a plus for us women. for me, who's not into material things, it's the thought that counts.i envy you wife for having a husband like you. even asking this question is effort already!

How about hiring a maid service to come in once every week or two for a full year to help keep the place clean and free up your time to get out and do things together.

ok. loved the limo idea and vacation idea and the maid/cleaning lady idea. but what i would like this year are coupons for things he will do for me, chores that i decide on. For instance i would use one for cleaning out my car, cleaning out the garage, help with my flowerbeds, etc.. things i cannot get him to do on his own. unless you happen to be one of those who keep these things done without the nagging first ;)
