I have always heard it refered to as "having your mind in the gutter" instead of your head - probably the same thing though.
Usually refers to impure thoughts of a sexual nature, and often has little meaning outside of the context of the situation that triggers the thought.
Also, you might just ask your friend what he thinks he means when he says it. If he doesn't really know, then you might point out this most common meaning, so that when he says it, people who don't know him so well don't think he is some kind of pervert.

This is an old saying when gutters meant the drain that runs along a street and under. When I use this saying it's telling someone that their are thinking dirty or nasty. Thinking about a dirty thought. I hope this helps.

Head in the gutter is just code for "dirty mind". If you said something like "Sure, I could get behind you on that..." and your friend started laughing and said "Sorry, my mind is in the gutter", then he's just saying he heard what you said and thought of the sexual innuendo instead of what you meant.

I've never heard "Head in the gutter". Often heard "Mind in the gutter".
It means you are twisting the meaning to have a dirty meaning that wasn't intended.

gutter is a slang term usually used to mean a place that's dirty... or sometimes even a place where you'd go to get laid... if your head is in the gutter, you're thinking crazy thoughts usually involving sex or seepage or some other wild and crazy activity with a fluid transfer or two

If your friend is literary, that may be what he means. If not, tell him to watch out for the street cleaning machine.

Usually refers to thinking dirty (ie sexual) thoughts. Example what do you think of when you see a pair of apples with a banana in between them? If your not thinking fruit bowel or breakfast, your mind is probably in the gutter.
