I grip so there's about 1-1.5 inches of shaft skin between the top of my hand and the head (index finger and thumb right over the circumcision line). Then I slide the skin up and over the head so it just folds on itself over the rim. That folding and unfolding over the edge of the head is what works best for me.
So you could say that I start by gripping the skin at the middle of the shaft, but do the actual jerking right at the rim of the head.

Higher with the right hand and lower with the left hand. They work together.
