Some of the more obvious reasons would be that they are unable to cook, cannot cook, forgot about it, set the heat too high, etc.
Of course, more knowledgeable people can say that heat carries over when food is cooked, so sometimes people remove things they cook early because the leftover heat inside the food will continue to cook it.
Also, some may prefer the taste of charred or near-charred food.

They become distracted by somethin or someone else; a friend, the telephone, or they decide to walk away as the food is taking ages to cook but forget about it till it's too late!

They are trying to do too many things while cooking. Some people can do this, others burn at it.

Distractions mainly I would say. I always double check though since I use gas and I have a fire extinguisher in my kitchen. However, if food is burnt while they are watching it, they have the temperature all wrong. That is a major cause in my opinion.

They're gossiping on the phone and forget they have something on the stove. Or they have to make an emergency run to the toilet and forget to turn it off.
