I think is meant to indicate you have a mental problem in a joking sort of way! There are other funny sayings too:

His elevator doesn't go all the way to the top.
He is a few bricks short of a full load.
Lights are on but nobody home.

If anybody says that to you, don't take it literally!

Definitely NOT a compliment. It's like saying, "You're not the sharpest knife in the drawer." "Lights on; nobody's home." Not nice.
