I would seriously consider doing west OR east, not both. There really is so much to do in the four locations you have mentioned, I think you might feel frustrated at having to do everything at such a breakneck pace. New York is fantastic, amazing, unique and the last time I went I stayed for two weeks and it STILL wasn;t nearly long enough. However if it is your first time in the USA, I think the west coast might be a good place to start. Hire a car. In 18 days, you could fly into and explore San Francisco, spend a couple of days in Yosemite (spring is best - the waterfalls will be incredible!), drive down the Pacific Coast, stopping at beautiful places like Carmel and Monterrey, spend time in LA (I don't like it, but everyone's different!), drive out to Las Vegas and Grand Canyon and fly home from Las Vegas. Adding in New York to this already full itinerary means you won't do any of the places justice, and they really do deserve it. Hope this helps.

There is one part of this trip that you should definitely rent a nice car (A convertible) and drive down the coast highway of California (known as hwy. 1). Stop off at some of the the many vineyards you will be near and be sure to visit the Hearst Castle at San Simeon. The road from Los Angeles to Las Vegas is notably unspectacular and you should fly on this leg. Also the Grand Canyon is only 5-6 hours drive from Las Vegas and its worth the time. Drive down to Phoenix from there for a return flight to New York.
If you would like to hire a tour guide with impecable integrity, I'm available for a small fee for the driving portion of the trip. email to my profile for inquiries.

I would say go onto the websites of various airlines in america - American Airline,Delta,Continental etc and click on the multi flight option. Most of them have this facility.
I would suggest fly to San Fran, then to LA, you can drive from LA to Vegas (about 4 hours) and then fly to NYC.
Or in reverse.
I have just looked at American Airlines website, and put in details for January and made up this itenary - should be similar in April
10Jan - London Heathrow 8.30am to New York JFK
15Jan - New York JFK 8.45am to San Francisco
21 Jan - San Francisco 09.45Am to Los Angeles
28Jan - Los Angeles 19.05pm to London Heathrow (lands 1.30 pm 29Jan)
Didnt bother with flight from LA to Vegas, hire a car.
These flights only cost 440 pounds including taxes.
Might come along with you at that price !!!
