The way Combat Level is calculated, they take only the highest of melee(attk/str), range, or magic to determine your "offensive power". Defense, HP, and Prayer always affect your combat level, but only your "most powerful" offensive combat skill is used to figure out your combat level.
So, if you are trying to get to lvl 126 combat, you don't ever need to train range or mage if you don't want.
On the other hand, if you're trying to make a "pure" with low combat level, you can train mage as a side skill to range or strength without raising your combat level.
See the link for a combat level calculator:

It depends. For people with magic and ranging reasonably worse than strength and attack, it is attack, strength, defense, hitpoints and prayer. For cases when maging is better, it is the same as melee except mage instead of attack and strength. For cases when range is best, it is still the same except range instead of mage or attack and strengh.
P.S. I am a meleer.

It's all of them. The more potential you have for any kind of offense - stats, skills, magic, archery, swords; they all increase your level of combat. It's like that so nobody can be everything, lvl 100 mage, lvl 100 swords, lvl 100 range, that would just be incomprehensibly difficult, like 50 billion exp points or something crazy ;p
My runescape terminology might be a bit off, I quit playing it when they made the new 3d engine, but I remember that. (nobody email me asking for my account, cause I already gave it away)

All of the above. Any skill that has to do with combat will affect your combat level.
