Easy, burger is the shorter form of hamburger. We get hamburger from the place called Hamburg, Germany. This is where hamburgers orginated. To be a "true" hamburger, there needs to be ground meat. The Filet-O-Fish does not contain ground meat (its a filet). Also, chicken sandwhiches are not burgers, because again, there is no ground meat. A sandwhich is anyfood placed on or between two pieces of bread. It was named after John Montagu, 4th Earl of Sandwich. (I'm not joking)

Burger refers to the meat. You can have a burger without the buns.
You should be asking. Why is it called a hamburger when it's made out of beef. Shouldn't it be a beefburger?

Over here we call them crap.
I think the 'burger' bit, for us uncouth Americans, implies chopped or ground meat. Nobody wants a fishmash sandwich...
