Probably not - especially if you're in an elevator. They're jumpy already about the close space. It's acceptable in the US to use "How are ya'?" in place of Hey or Hi. A stranger is probably greeting you, and not demanding information.
It's up to you if you want to take it as a shallow dodge, or answer them anyway to remind them they DID ASK. ("since you asked, ...)
People suck sometimes and then, sometimes they surprise you. Might as well be prepared for the worst and have fun trying.

While some people are genuinely interested, most are only asking to express a certain level of courtesy to others while they hurry about their own business.
One should never expect a casual acquaintance to actually take time to listen to any answer other than "well", "fine", or even " I've been better." No further explanation is necessary or desired!

people who greet you are doing so to be proper and to acknowledge you as a person in their path,... those who frown are the ones who couldn't actually give a **** about the answer...
it's actually healthy to smle and say "hello" or give a greeting of some type... does a person good, and it's good for our confidence, too
... going around frowning with our heads down all of the time shows a person is unsure of himself and probably lacks social skills.

when u meet someone sometimes it is very difficult to start with conversation. so we start with "HOW ARE YOU?" so that the conversation began and then u can touch various subject....
similarly it is a mannered way of talking with anybody.
