A. Fire -- I've always had a horrible fear of fire. A friend of mine was terribly burned in a car accident several years ago. She survived, but endured terrible pain and many surgeries... for years, she also had to deal with emotional isolation as rude people would stare at her due to her scares. The pain she endured made me realize what a torture dying from fire would be. I wouldn't wish that on my worst enemy.
I almost drowned once... I felt a horrible sense of panic but no pain. There was a point -- before I was rescued -- where I began to get tired from fighting and could see how I could just choose to just go.
Being eaten alive would be bad, but I imagine that in most cases it would go quickly because hunters tend to quickly kill prey.
With "D," I can imagine a huge sense of panic and helpless waiting as in the drowning scenario. However, I've heard that it's not the impact that kills you but rather a heart attack. That seems merciful.
Cancer, without question, is awful. Thankfully, however, we are equipped with pain medications, grief counselors and other coping mechanisms. There is also a silver lining in that people know it advance to make peace with friends, loved ones and take care of personal issues.

All the above. First I'd get cancer, then I'd be riding in an airplane over a ocean, the airplane crashes and the shute won't open cause it got stuck on my leg, then a animal that was in the storage breaks the floor up from below and tears me up, the water from the sea starts filling up the whole plane, then the gas tank catches fire and the whole plane blows up. =P

Aside from Crucifixion?
Fire. Burns are, inch for inch, the most painful injury a person can sustain. Drowning is supposed to make one feel sleepy and content once they've gotten over the initial panic of trying to breathe water. Skydiving accidents are almost instantaneous death. Cancer is debatable, but physically can't be as painful as full body burns. Being eaten by an animal would suck but still... inch for inch burns have it.

Terminal Cancer - because it is slow, painful, predictable but
impossible to correct. Not only does it rob you of your life but it also inflicts pain on those loved ones who have to see peoples lives deteriorate. It can also take away some dignity from the person suffering.

i am not certain what the most painful out of all of those listed would be, but i would have to say E for most people.
As it is, i have this insane fear of drowning, even though i can swim. So for me, it would choice B, although ALL of those listed would kinda suck. As for choice A, you wouldnt burn alive. You would die of smoke inhalation before you ever were burned.
