You should be thankful your son has a teacher who cares enough about him to actually correct his work. Elementary school lays the foundation for the rest of your son's life. It's great that he gets all the answers correct, but if he can't follow instructions, like "make sure you underline your answers" then he has missed a little piece of the puzzle by not following an instruction.

It probably seems pretty insignificant, unless you've ever taught middle school or high school and had students who can't even fill out a simple questionnaire, much less complete an exam or assignment correctly. Following instructions and presenting neat orderly work is important.

Anybody who claims differently is probably incapable of doing it themselves!

I agree with one of the previous answres. If he isn't following the simple directions, like underline not circle, he may have an issue following complex directions if he is not aware he is making a mistake. Maybe have him start underlining the word underline or circling the worg circle in the directions. If he is being sloppy, have him practice handwriting at home.

(On a side note: My school is all about quality. If a child gives sloppy work, we are supposed to return it and make them redo it. He's lucky he doesn't have that issue, but it may become something he will face in middle and high school. It's good to start encouraging him to show pride in his work.)

The teacher is making sure your son is following the directions. Case in point, what happens when he goes to vote for President? If he circles the name of who he is voting, instead of checking the box (or whatever your state does) his vote will not be counted. She is trying to get him prepared for a life of following directions (SAT, State-wide test, Voting, etc....)

If you have not talked to the teacher about this yet I would suggest setting up a parents-teacher-student meeting to help figure out a solution for surving the remainder of the school year.

it seems like your son is understanding what he is learning well enough to write an A paper about it. perhaps his teacher is just making him aware of some grammatical corrections so he can get them right the next time around. Grades are not as important as the experience of learning as a whole

I had the same problem whilst in primary school. I'm now completing a PhD at the age of 24. I think the problem is rather trivial; you should not concern yourself with trivial matters.

She sees he is good and may want to raise him to the level of perfection, or perhaps she sees him not taking time to do something correctly. Either way give him praise from your end and tell him thanks for doing great.

Perhaps the teacher doesn't like your child and is picking on him. Or, they know he is bright and they are trying to make him do his very best. It's hard to say but I would schedule a meeting with the teacher to find out why. It would be sad if this would make your bright child not like school.
