I go by a name that isn't on my birth certificate. However, it is on my baptismal certificate and throughout my life and school people refer to my name as the one in my baptismal certificate, which i like and am accustomed to. i actually use the birth certificate name as a middle name.... for example: roger [baptismal] paul [birth certificate] then mother's maiden and then last name. --those are just example names by the way..heh. my baptismal certificate has the full name that i would like to have. college apps are around the corner and i'm confused on what to put for my name. no one's ever really called me by my birth certificate name except for a doctor.

I have a friend that did the same as you....when he filled out forms he would put the name he wanted to go by in quotation marks ...example.... John "Paul" Jones. That way the person reading it will know what to call him.

You can also petition the court...or file for a name change to make it whatever you want.
