I think if it gets to "fool me the third time" the saying changes to "I'm obviously a damned fool!" Twice should be enough for anyone. Fool me once, shame on you means -hey, that wasn't nice. Fool me twice, shame on me means - oh, I should have learned from the first time you lied/cheated/stole, etc. Fool me three times - that just makes the person who was "fooled" an actual, real-life fool for sticking around with a liar/cheater/thief
