It's "an"... You use an if the next word starts wtih a vowel, OR if the next word SOUNDS like it starts with a vowel.

it's "an MRI"

The English language was structured so that if you have a noun that begins with a vowel or what sounds like a vowel, you put "an" instead of "a" before it--this makes speech flow better, so that you don't have any "ugly" pauses between words. If you say "a MRI" out loud, it just doesn't sound right.

When using acronyms, they themselves often become commonly recognized words.
Using the full name would require just "a."

Eliminating the combination of letters that form the acronym, you would use "an" when the subject is the letter itself whose sound begins with a vowel. An "M."
