It looks as if his insides are comming out (from where he poops). He also looks dehydrated. We fed him carrots for the first time, could this be the cause of it? He has been sleeping for days, he moves sometimes, what should we do?

This definatly sounds like a prolapse. Prolapse is caused by a calcium deficiancy, it causes his rectum or hemipenes(sexual organs) to protrude outside his vent. Do not try to push them back in, it can do serious damage. Take all substrate out of his tank, and use paper towels. The prolapse can prove to be lethal. It is generally not the prolapse that kills, it is the infection that it causes from the internal organs being outside the body.
Keep it very clean, and get him to a vet. If the organ goes back inside his body on it's own, then all the vet will have to do is give him an antibiotic in case of infection. Give him a calcium supplement in the meantime. Make sure your UV light is recent. This bulb needs to be replaced every 6 months, if it is too old, he is not getting the proper vitamins from it. Sprinkle calcium dust on his food and his crickets, this will help for now.
But if he has an infection from it, only a vet can make that better. Prolapse can kill in a matter of 4 days from the time of protrusion... so time is ticking, get him to a vet ASAP

He could have a rectal or hemipene prolapse. I'd get him to an experienced reptile vet ASAP. Keep the area moist until you get him in (you can use K-Y jelly on it). Make sure he is not on sand or other loose substrate-- a common cause of impaction and straining. Parasites are another cause.
