I am not always aware. People sometimes feel pain because of their own ideals. That is to say that what affects a person one way may affect the next person in the other. Sure, sometimes it is obvious. If I am aware that someone is hurt by my action, my reaction will depend on wether I was right or wrong, or if that person was not just 'being himself' while I was being mine and then taking unneccessary offence. If I do find that I hurt someone in a wrong way I think that it would be in order to seek to 'make amends'. I really hope that I would try to apolodise and do just that. Admittedly, there have been regrettable times in the past when I have failed to 'deal with' my own faults and like just wished it away. I never feel good about that. I think I would desperately seek to make amends. Once bitten twice shy...

As I got older, I became more attuned and empathic with other people's feelings.
In a more realistic setting, the answer is no, you are not always aware of pain you inflict in the same token that No, neither is everybody always aware of the pain they can inflict on you sometimes.
Words, and acts are double sided swords. They can wound in every direction.

If someone acts hurtfully, he does so with intention. He may not be conscious of his intention, but that in itself is a defense mechanism. A taking of refuge. A hiding out. If exposed, he will do the "It's beyond me" routine.
If a person stops hiding from himself and others, and becomes aware of how he has deliberately hurt others, he can then forgive them and ask their forgiveness. And make amends if necessary.

Can anyone really be fully aware of how we affect other people? I can remember when I was young having something mean said to me that stuck with me for years but I also remember nice things that people have said to me that I don't think they have any idea on how much I think back on and profoundly appreciate what they said. Also when we reach to someone ,for better or for worse, how those words can affect others that they themselves reach.

yes.. never fails..
there is a reason why i can sometimes hurt someone.. it is not for the fun of it, but it is a reminder to let them know that i am aware that they wronged me...i dont usually cause the pain.. i just tend to throw it back at them...

Now that I practice Krishna Consciousness I try to be very careful of my thoughts, words and actions as not to hurt others. Including animals, bugs etc. I consider that there is an eternal soul within the body of every living being and it is not my right to harm them. I can only try to protect myself if they attack or do my best to protect someone in my presence who may be being unjustly attacked. Being Vegetarian is a very important part of practicing non violence. Otherwise it is just a type of hypocrisy to say not to harm one species and harm another. As far as plants, they are for food for human beings but they food must be offered to God (blessed) so the soul will get the blessings to get a higher birth in their next liefs existence. But God does not accept animal sacrifices. Do you wonder why a child is born sick or blind and in a poor family or healthy or in a wealthy family, that is due to past life actions (karma) We will get the pain we deserve due to our past actions. One must understand Karma, Reincarnation and no one goes to hell eternally to understand the unlimited mercy and love of God.

i should be an expert on this by now, but i'm not. i sometimes act without thinking, though i try not to. we all hurt someone, sometime. we just have to try to be aware of that, and try to do better.
