"Facilitated" means to make things easier.

Regular diffusion through a plasma membrane means that materials just slip through the membrane in between the phospholipids.

Faciliated diffusion happens when materials, for whatever reason, can't slip between the phospholipids but have to have a kind of channel to be able to get through the membrane. They go through special channel proteins - kind of like a passageway to get through the membrane, but still without using any of the cell's energy.

So the match for 3. facilitated diffusion is A. diffusion of molecules through a protein channel.

1C
2B
4D Against a concentration gradient means to go from low to high concentration instead of the normal diffusion way which is from high to low concentration.
