Qantas.
Though the statement is no longer correct, Qantas failed an engine in 2004 during a flight to Vietnam. No one was injured.
BUT if you don't call that crashed, it's still true...

If they were only flying to Austrailia then that would have been a great piece of information.
Perhaps they should have done that. Fly to Sydney and then to LA.
