Yesterday at Wal-mart I saw the cutest thing. You can get those glass prisms (that you put on a machine and they light up and turn) with a picture etched into it!

I know you said something with a picture, but LL bean has a storybook that allows you to personalize the whole book, and each page starts with a letter of the baby's name. We got this, and we thought it was soo thoughtful and very personal and special for our baby.

Do you know that Asda's (Wal-Mart I think if you are in US) photo section now do these kinds of personalised gifts - they are great!
You just hand the photo into them.
Theres mugs, bibs, canvases, tea towels etc.
