1. Slap the neighbor that's lived across from my parents for like the last 15 years;
2. Take out a 1.5 million dollar life insurance policy that will actually be paid out upon my death;
3. Pick out the funeral home, cemetery plot and service;
4. Revise my will and make sure my nieces and nephews have trust funds; and
5. Tell the people that I love and care about that I do in fact, love them.
Not in that order, of course.

It would be too late for me anyway. By the time I get my fibroids out from my body and so-called benign growths removed, the results will tell me that I am going to die due to the number of years that I had them. So, it wouldn't matter much what five things I want to do before I die, because I will dying anyway, and NO ONE, not even Tobey Maguire, would care.
