Wearing your heart on your sleeve means that the person is open and honest about their emotions. They don't mind letting others see if they are happy, sad, in love, etc. It makes them vulnerable, in a way, because they can be easily misunderstood and easily hurt.

It's rather more gauche than simply being unable to contain one's emotions. The phrase is usually used to describe a romance in which one party in a variety of ways reveals the object of his love to others. We notice usually because the love is - alas - unrequited. (Often because the lover has been so artless in his demonstration of love). Most often, it makes the heart wearer either an object of pity or scorn - or both.
Moral: Keep your love to yourself until you're somewhat certain it's returned.
