When I was a dancer, once I got so drunk at work that when it was my turn to go onstage, my dress was inside out and I didnt notice. Another girl pointed it out and I just shrugged and took it off about 2 songs early.

I went to my spa and got a Brazillian... I wore a dress and when I walked to the front desk to pay, I felt a draft... felt my butt and realized my dress was in my panties. Worst part was that someone was sitting on a chair behind me and if you look into the front window of the spa, you can see straight in.
That's one my accidental wardrobe malfunctions. There have been many that I didn't realize were wardobe malfunctions while wearing them.... like... well, let's just say I made some pretty bad fashion mistakes.

YES!! I was on the tennis team in middle school (7th grade) and there were two 8th grade boys that I liked that were on the team as well. So one day the whole team is going to practice but someone locked the gate to get to the court so one guys was helping me over, by giving me a 'leg up.' I got one leg over and stuck in the chain link fence and I said to the guy 'okay I'm good you can let go' (he still had my other foot) instead he gave it a good PUSH and it went up over my head (so I pretty much did the upside down splits in the air with one foot still stuck in the gate) and my pants SPLIT right down the middle! And I mean a BIGG rip! They were a weird material, not like jeans. Talk about wardrobe malfunction!

For an easy credit in college, I took a swimming class.
We were sitting on the bleachers by the pool and I looked down. What did I see, but both my "boys" hanging out through a rip in the lining that I didn't realize I had.

At my 9th grade prom in Junior High I was wearing a strapless dress. I danced with a guy who was pretty tall, and I had to reach my arms up high to touch his shoulders (I am only 5'2"), and when I did, my dress sort of came loose at the top and oops! There went my boobs! I wasn't wearing a bra either. : ) Thank goodness he was the only one who saw! I would have died if the whole school had seen!

track in high school running the 1/4 mile--jock strap broke midway through and finished the race 'dangling' out one leg. I got a standing ovation.
