Everywhere because everyone feels lonely sometime or another. It's not right to classify someone as always lonely, it's an emotional process. If someone is feeling lonely all of the time even when not alone, this person may be experiencing clinical depression. Everybody's lonely sometime.

From many different places but often from circumstance - i.e. OAPs living on their own, after the death of their partner, isolated and feeling alone especially during winter.
It certainly isn't inherited - how would that work.
