Just curious if this is something that can happen to newer viehicle typically due to wear and tear or if there must be something defectvie before it can come off.

bad motor mounts
the mechanic that put it on left something loose
failer to properly lube the u joints
jumping your car/truck
broken bolts
it shouldent fall off on its own something usually is the cause the list is long

Depends on how you classify new. If you have a vehicle that is only 1 year old but has 60000 miles on it, is it still new. There are some parts like tires and break linings and even universal joints that need replacing by this mileage.
My answer is generally it is defective if it fails before its projected life span.
It is possible that if something was thrown up under the vehicle it could damage the drive shaft causing it to fail too.

If you are talking about a new (or fairly new) car, it can't be wear and tear. Somebody at the factory probably didn't tighten all the bolts.

Drive shafts don't normally just "come off." Unless there was a U-joint failure (i.e., broke apart), it shouldn't come off. If yours did (and there weren't broken u-joints), then there were parts installed loosely or weren't torqued correctly and worked their way loose.

Simple the driver caused extreme abuse or it was defective out of the gate.
