I'd say Boca Juniors v River Plate, or the El Classico (Real Madrid v Barcelona). The atmosphere is electrifying in these two derbies, though I won't recommend that you catch a Boca-River match if you want your hearing after the match. I'd rank these two ahead of Liverpool v Man United.

At the team level, I'd say there are several: the most obvious one would be Internazionale v Juventus -- it's not for nothing that this is called 'the Italian derby'. (Speaking of Italy, let's not forget the city rivalries, especially Inter/Milan, Roma/Lazio.)
Although I've never been to Argentina, I've heard that Boca v River Plate is special. The photos I've seen of it certainly are amazing.

For me, no.
Though I went to a Celtic vs. Rangers game many years ago and I was amazed at the atmosphere. Definitely recommended.
