Honestly? What to get my stepmother for Christmas. Then what to do with my dogs while I'm gone to the inlaws for a week. Then where I can find the best vendor for a bunch of lab equipment I need for work. So those are my top three concerns right now, as an American.
If you mean what's the top political issue that concerns me I would say it's the constantly expanding government. Every single problem you can possibly face seems to be something the government feels it has the power to influence. The race has come down to who can spend the most, promise the most, and do everything for everybody forever. And then people are surprised when the government erodes on their freedoms! I swear the vast majority of our country is downright retarded. You can't have it both ways. You either get a large intrusive government that does a whole bunch, or you can have a small government that doesn't do quite as much as you might want. I personally would like to see our government slashed to a skeleton crew. My money, and my freedom, is more useful kept in my own hands.

MP US Army has it exactly right. The biggest concern for every American should be the overreaching arm of the government and its total disregard for the limits placed upon it by the Founders and further codified in the Ninth and Tenth Amendments to the Constitution.
By the way, the text of the Ninth Amendment:
