Often, websites have an automatic recovery mechanism for folks who forget their passwords. A very common term they use is"reset".

So to reset a password means you answer a few security questions correctly and they allow you to create a new password or email you a new password.

After successful completion of this process, you would say that your password has been reset.

Depending on the application, sometimes if you attempt to login several times and fail, the app will automatically reset your password, locking you out of your account. At which point you need to follow whatever policies / instructions they have in place to unlock the account by proving your identity.

Basically it has either changed (reset) or is a method of changing, your password.

Depends on the circumstances. Certainly a strong indication that the account has been compromised.
