This is pretty obvious, but its true. People died, it was a war.
Lack of food.
Supplies had to be pushed toward the front to feed the military, though they often lacked food themselves. Then came the destruction of cities and transportation and it got harder to get food places.
Suspicion of spying.
Both sides were worried about traitors in their midst. It wasn't too big, but it was still something to watch for.

(1) brothers would wind up fighting against brothers because one believed in slavery & the other did not
(2) northerners generally didnt know anything about being soldiers so they wound up with alot of accidental deaths--even injuring themselves
(3) slaves had to choose whether to join the fight or not because if the south had won any slaves that fought on the north's side would be stamped as traitors

conditions were not good, so food and starvations happened thru soldiers. Poverty was a big one, especially in the south.
