It basically happens as First Dragon described, but it is important to understand that fertilisaton happens inside the chicken before the shell is formed in the chickens reproductive tract. The shell is gradually deposited after fertilisation as the egg travels down the oviduct.

The rooster stands on the hen's back and rubs his vent under her tail on her vent. Birds don't have a penis; the sperm just move on their own up to the egg inside the hen and fertilize it.
