You have two options:
1. Take your system to a reputable repair shop
2. Go to a HijackThis forum and have an expert vollunteer walk you through cleaning up your system.

Go visit several of them, you can read as many posts as you wish without signing up. When you find one which seems to fit your needs, where you feel comfortable, sign up. There is always a "sticky" thread with steps for a beginner to follow. It involves downloading a program called HijackThis, running a scan and posting the results back to the forum for evaluation.
Here is an MSN group called Freeware and Shareware which actually also has a chat room feature where you can actually have an expert remote into your system and fix it for you if you find it too difficult to do all the steps.

I really like this group, and I trust the members with my own system. I now do most of my own repairs, but these folks really helped me out a time or two, and you can easily see how they help others too. They do this for free and around their own personal responsibilities.
Please note that these are experts in the field and if you were to walk into their offices or shops you would have to pay some hefty bucks for what they provide for free online.

Consider this an excellent time to recognize that
your operating system is defective. I recommend replacing
it with one of:
http://fedora.redhat.com
http://www.centos.org
http://www.opensuse.org
http://www.debian.org
http://www.ubuntu.org
http://www.slackware.org
http://www.freebsd.org
http://www.opensolaris.org
Any of these will be more reliable, less susceptible to
viri, uses your hardware more efficiently, and they're
all free.

if it finds some files and won`t delete them (due to commercial reasons that might happen) copy&paste the report on the notepad, search the files and delete them. don't forget to unable your antivirus temporally before online scanning
